Use cover to stop Shiv landing repeated Serrated Knives, which add bleed stacks and refresh their duration. Move sideways as Slice and Dice starts. Bloodletting delays some damage, so keep your distance while it takes effect. Hit an enemy hero with Scorn when you need its stronger healing. Use Sand Blast against gunfire; it does not stop ability casts.
Recover health before returning to a fight where Killing Blow can execute you. Dodge Slice and Dice and attack Shiv from range while you are wounded.
